Job Description

Responsibilities:

  • Conduct holistic case management, including assessments, care planning, interventions, and care evaluations to support patients across campus and community services.
  • Develop, facilitate, and conduct group work sessions to build social support networks for targeted patient groups.
  • Collaborate with community partners to assess community needs and implement programs to enhance patient psychosocial well-being.
  • Participate in developing and implementing programs to enhance the quality of care and social work services.
  • Provide guidance, supervision, and training to junior social work staff, assistants, and social work students.
  • Conduct psychoeducational talks, workshops, and seminars for patients, caregivers, and the public.
  • Support research initiatives, including data collection, analysis, and development of presentations or publications.
  • Assist in managing department services, resources, and projects to achieve quality indicators and department goals.

Requirements:

  • Degree or Postgraduate Diploma in Social Work.
  • Fresh graduates must have completed at least 400 hours of supervised field placement.
  • Experienced candidates should be eligible for Social Work Accreditation Board (SWAB) registration or already hold Registered Social Worker/Provisional Registered Social Worker status.
  • Strong understanding of social work ethics, values, relevant legislation, and multi-cultural socio-political contexts.
  • Good interpersonal skills, with the ability to work collaboratively in a multi-disciplinary team and with community partners.
